Gwernllwyn Care Home is situated in the village of Cross Hands, Carmarthenshire. Although a rural area, it is well served by major road systems and is conveniently close to the M4. The towns of Llanelli, Carmarthen and Llandeilo are less than a 30-minute drive away and are easily accessible. The village has a variety of Supermarkets, convenience stores, shops, eateries, coffee shops, pharmacies and a post office and some interesting local attractions. Gwernllwyn Care Home provides residential care for men and women. At Gwernllwyn we can accommodate no more than forty-one (41) persons. Up to 41 places for people aged 60 years and over requiring personal care. Up to 21 places within the 41 for people ages 60 years and over living with dementia requiring personal care and 1 place for a person aged 55 years and over requiring personal care. Our first floor is known as Ty Pilipala which is home to individuals living with dementia. Butterflies represent endurance, change, hope and have a powerful representation of life and its transformation, whatever the stage. Gwernllwyn Care Home aims to provide a service which supports individuals to achieve the best possible outcomes in respect of their individual needs. We do this by providing a care and support service which meets the assessed needs in a way that the individual prefers. We also risk assess and mitigate, as far as possible, any risks associated with meeting the assessed needs. Our overall aim is to support individuals to be as independent as they are able to and wish to be, for as long as possible. We offer long term, respite and day care. At Gwernllwyn, we understand that individual’s needs change and fluctuate, therefore, we adopt a flexible approach to the support that we offer and accept that continuous review of needs is essential to support peoples changing needs. We are proud to practice person-centred care at Gwernllwyn. This approach will always be the foundation of the care we provide. What we do is dependent on each individuals’ preferences, needs and wishes, which empowers individuals to take control and make them the expert in their own lives. We ensure that each person’s preferences, needs and values guide decisions, to provide care that is respectful and responsive.
